Hope Hospice and Community Services is a not-for-profit, 501 (c)(3)-status
community-based health care agency serving Lee, Hendry, Glades, Collier,
Charlotte, Highlands, Hardee and Polk counties. We provide a variety of
services to all people with complex needs related to transitions, loss
and end of life.
Our programs
include:
Hope
Hospice - enabling seriously ill patients and their families
to experience the final stage of life together, in a setting most comfortable
for them.
Pathways of Hope - our counseling program, provides grief
support to patients, families and the community. It also provides support
to schools, businesses, law enforcement, health care professionals and
other community organizations. In addition Rainbow Trails Bereavement
Camp for children helps youngsters to cope with the loss of a loved one.
Hope Life Care - is a Medicaid waiver program that provides
home and community-based services to people age 65 and older with limited
abilities that qualify for nursing home placement.
Hope Connections - serving seniors age 60 and older,
providing help with daily activities including personal care, meals, transportation,
medical equipment and supplies, and emergency response systems. 
Hope Adult Day Health Care - for adults with cognitive
and physical limitations who require daily assistance. It provides a structured,
protective environment and therapeutic social and health activities. The
program takes place in Fort Myers at the Hope Health Center.
Hope Select Care - enables seniors age 55 and older to
live as independently as possible by providing all-inclusive services.
The Medicare/Medicaid program also addresses participant's unique medical
needs, and helps with the social, emotional and practical challenges that
older adults often face.
Hope Child and Teen Care - provides a variety of services
for children and their families including pediatric hospice care, palliative
care services, and grief support and counseling.
Hope Comfort Care - a consultative program for home-based symptom management
services to those who are living with a serious illness.
