The many symptoms of illness and the effort to “put on a brave face” can make daily living more difficult, affecting quality of life. Hope can help.
Better Quality of Life
The Hope Comfort Care team has the experience and knowledge to provide exceptional, person-centered care for people with serious illness. Hope’s assistance can:
- Decrease medical costs
- Reduce hospitalizations, including ER visits
- Reduce symptoms such as: anxiety, depression, pain, fatigue, sleeplessness or shortness of breath
- Improve comfort
- Enhance quality of life
- Address caregiver needs
Hope Comfort Care services can be provided in a private residence, Assisted Living Facility, Group Home, Nursing Home, or in our clinic.
Addressing Concerns
The first step toward improving quality of life is addressing the concerns and preferences of both the individual and the family - to determine what help is needed. The Hope Comfort Care staff is available to discuss:
- How You Feel: What hurts - both physically and emotionally?
- Daily Challenges: What tasks are hard to accomplish?
- Medications: What medications are taken, when and why?
- Environment: Would the support of medical equipment or other devices be helpful?
- Medical Planning: What are the goals for care? What is Advance Care Planning?
- Financial Concerns: Are the medical bills confusing? Are there programs that can help?
Individualized Care for the Person and Family
The Hope team works together to care for the whole person and their family. This includes:
- Specialized physicians
- Advanced Registered Nurse Practitioners
- Nurses
- Social workers
- Chaplains
- Personal care specialists
- Comfort Care Volunteers
Coordinating Care
Scheduling Assistance: People with serious illness tend to have full calendars - appointments with physicians, specialists and therapists. Scheduling can seem like a full-time job. Hope can coordinate these appointments and assist with necessary care.
After Hours Support: Benefits include a 24-hour call line for support at all hours and private duty nurses when needed.
Transition Services: During an illness, a person may find it necessary to move from the hospital to a facility or return to their home. Hope can help support this transition by:
- Suggesting appropriate housing options
- Helping with necessary paperwork
- Coordinating travel
- Explaining the roles of the new care team
- Making arrangements for medical equipment and safety aids at home
- Creating a home care plan - to schedule medications, therapies, follow-up appointments, and meals
Special Services: Support may also include psychosocial and spiritual counseling, caregiver classes and support.
