Hope Comfort Care is a consultative program for home-based symptom management services to those who are living with a serious illness. It provides medical services in the home, care facility, or retirement community to those with serious or chronic illnesses, age-related health conditions or illnesses that limit the ability to fully function. It supports those who may recover fully and those who are experiencing a progression of their disease.

A unique Comfort Program is created for every patient, based on individual preferences. Our team will help clarify your needs and goals, discuss treatment options, and manage your symptoms. Your Comfort Program may evolve over time as your needs and goals change. Hope staff will communicate often with you and your healthcare provider - an essential component of creating an optimal plan of care. With Hope Comfort Care, you and your family will always be included in decision-making and planning.

Managing Symptoms
Treating symptoms is important. For example, unrelieved pain may cause you to experience feelings of depression, anxiety or helplessness, sleep disruption or loss of appetite.

In addition to pain, other symptoms of chronic or serious illness include:

  • shortness of breath
  • fatigue
  • nausea and vomiting
  • constipation
  • skin irritation
  • and other problems

Will I still keep my regular physician?

Yes. We will check with your physician first to determine if a Comfort Care consultation is the right step for you. Your doctor may welcome the specialized expertise and experience of Hope’s Comfort Care Team. During the consultation process and any follow-up visits, Hope works with your own physician and other healthcare professionals to help you receive the care and treatment you need.

Where can I receive Hope Comfort Care?

Our staff is available to meet with you in your home, retirement community, nursing home or assisted living facility.

How do I pay for Hope Comfort Care?

Medicare, Medicaid and most private insurances cover our consultations. We can also work with you to develop a payment plan, if necessary.
