Special
Care for Adults Age 60 and Older in Glades and Hendry counties
Through
Hope Connections, hundreds of adults in Glades and Hendry counties receive
special kinds of help in their daily lives. The program, sponsored by
the State of Florida Area Agency on Aging and Hope Hospice and Community
Services, coordinates services and resources to qualifying adults.
Services
may include:
Personal
Care and Medical Equipment
Clients with certain medical conditions may require specialized equipment
and supplies to effectively manage their illness. Case managers also provide
assistance and management of routine care.
Transportation
Mass transportation may be provided to and from clinics or physician’s
offices, community services, essentials errands, shopping, etc.
Nutritious
Meals
Home
Delivered Meals
Hope Connections helps the homebound continue to live independently by
delivering meals. These nutritious meals are essential for health and
well-being and are provided with the added benefit of daily human contact.
In addition to fighting malnutrition, home delivered meals also alleviate
loneliness, anxiety and depression.
Congregate
Meal Sites
For those who enjoy dining and socializing with others, hot nutritious
lunches are also served at Hope Connections’ meal sites, in a comfortable
group setting.
Personal Emergency Response System
This resource allows frail clients to remain at home alone more safely.
Homemaking,
Chores & Household Tasks
Our staff can arrange for household chores, including bed making, dusting,
dishwashing, vacuuming and more. We may also coordinate yard work, lifting
and moving, and simple household repairs. In some cases, home modifications
can be made to ensure older adult’s safety and well-being.
Counseling
Aging presents challenges such as the loss of loved ones and the loss
of independence. Counseling can help clients cope with periods of depression
and anxiety.
