Case Manager – Hope Life Care – Lee and Hendry Counties
Interviews and assesses clients to determine needs, monitor and manager ongoing care. Must have Bachelor’s degree in Social Work or a health related area – ex. Psychology, geriatrics, community health, etc. Case management experience preferred but not required. Pool is on an as needed basis but need someone approx. 3 days per week.#537 Pool

Master’s of Divinity from accredited school and three or more years of pastoral experience required. Must also be ordained and in good standing with a denomination. Chaplain or spiritual care experience in hospice or hospital setting preferred.
